Services to Children in their Own Homes (SCOH)

Wordsworth’s Services to Children in their Own Homes (SCOH) recognizes its place and responsibility in the continuum of care to children and their families. The primary objective of SCOH is to assure the on-going safety of the child in the family home while stabilizing and enhancing family functioning. Ultimately, maintaining children in their own home is ideal in terms of permanency planning. In order to accomplish this, Wordsworth works collaboratively with the family, the Philadelphia Department of Human Services (DHS) and other significant collateral contacts to address the goals as identified and developed through the Family Assessment Form (FAF) process and/or stipulated in the Family Service Plan (FSP). The purpose is to prevent any further incident of abuse and/or neglect and increase the family’s ability to appropriately handle both critical and non-critical life events. Each family presents a unique set of strengths and needs that greatly influence and drive the above process.

Wordsworth’s SCOH Case Management Services include:
• Family Assessments
• Monitoring of child(ren)’s safety and well-being
• Promotion of life-skills (parenting, resource management, meal planning, and wellness)
• Advocacy as it pertains to identification, acquiring, coordinating, and monitoring of community resources
• Coordination of services to plan, monitor, and document family participation in the total array of services

SCOH works with many other community resources such as schools and medical providers to help families achieve the goals set with those providers as well as DHS. Since providers often are significant members of the larger community in which families live and interact, they are considered an integral part of SCOH’s work with families.

Family Involvement
The family is the primary client for whom SCOH services focus. SCOH focuses on individual goals set in collaboration with the families to help children stay within the family. Parents’ and/or caregivers’ active involvement with SCOH are vital to the delivery of services. In order to provide the best service possible, it is expected that all family members are present at all scheduled meetings and provide Case Managers all information needed within a timely manner.

Professionals

The SCOH team brings together professionals from the social services and mental health fields and consists of Master’s Level Director and Supervisors and Bachelor’s Level Case Managers, dedicated to helping families remain together and developing healthier and more satisfying relationships. Wordsworth is committed to the continuous professional development of all employees. SCOH staff from the Director to the Case Managers are required to complete twenty hours of training annually through a combination of internal and external workshops. Ten of those hours are required to be on issues of direct social service and mental health practice. In addition to weekly supervision, SCOH staff meets regularly twice a month to review best practices and hold internal trainings.
