CASE MANAGER – BHRS                                                                                                   7/21/08

Responsible for the case management of assigned caseload, determining authorization of client services through client eligibility (Medical Assistance), communicating with families regarding scheduled appointments and ensuring continuity of care.  Responsible for the documentation necessary to complete authorization requests, and the development, timely delivery, and overall management of packets in order to successfully maintain client caseload.  Requirements:  Bachelor’s degree from an accredited University in psychology, social work, sociology, teaching or related field.  One year verified experience in case management or counseling with children and families, or is a licensed registered nurse with one year experience.

 

PSYCHOLOGIST                                                                                                                  6/23/08

Conduct Biopsychosocial evaluations/reevaluations of clients to determine medically necessary mental health services offered in the Wordsworth continuum of services or to recommend other service options.  Provide supervision and training of clinical staff and be available to consult regarding client care and direction of treatment.  Integrate clinical formulation for treatment planning purposes.  Collaborate with current treatment staff when conducting a reauthorization evaluation.  Requirements:   PhD or Psy.D in Psychology; must have current licensure as a psychologist in the State of Pennsylvania.  Minimum two years of post-doctoral experience in children’s mental health services.  Skills in completing biopsychosocial evaluations/reevaluations.  Familiarity with BHRS services as well as other children’s mental health services, managed care systems, and the Philadelphia mental health community.  Proficiency in the conceptualization of and interventions with childhood disorders including Oppositional Defiant Disorder, ADHD, PTSD, etc.    E-mail resume to lwlodarczyk@wordsworth.org or fax to the attention of Lee – HR at 215-871-3412.

 

 

SENIOR DIRECTOR OF COMMUNITY-BASED SERVICES                                               6/9/08

Oversee the ten programs, which comprise the Community Services division of Wordsworth.  The programs include BHRS, Family-Based, CARE-Finletter, CARE-Barrett, Community Placement Services, SCOH, BEACON, Consultation and Education Specialists, After-School Initiatives, and Admissions/Utilization Management.  This division of business includes behavioral health, child welfare, and school-based programs and candidates should be familiar with CBH, DHS, and the Philadelphia School District personnel who directly interface with the management and oversight of these programs.  Work with each program director to insure that quality service is provided to clients, contractual obligations are met, and reimbursements are received in a timely and efficient manner.  Budgetary development and oversight for each program in conjunction with the program director will also be a key role.  Responsible for implementation of all corporate policies and procedures, and oversee the training and payroll functions for all community-based programs as well.  Work with program directors to bring growth to current programs and develop new program initiatives as the opportunities arise.  Represent Wordsworth in the community and participate in organization activities (Alliance, PCCYFS) and at DHS, CBH and School District meetings and workgroups.  This position reports directly to the CEO, will be part of the Executive Management team, and will monitor the Performance Improvement process for all community-based programs and be part of the corporate PI Committee.  Requirements:  Candidates should have 3-5 years of supervisory experience in program management, a Master’s Degree in a social services discipline, knowledge of programs, and excellent time management and organizational skills.

 

 

HUMAN RESOURCES COORDINATOR                                                                           5/5/08

MENTAL HEALTH WORKER (CASE MANAGER) – FAMILY BASED PROGRAM

Work in conjunction with a Mental Health Professional in providing individual and family treatment, psycho-educational services, relief services, modeling and parenting training.  Must work effectively in a team environment. Work hours are non-traditional in nature. Requirements: Must have in-home and adolescent experience culturally diverse environments.  Experience providing services to families dealing with sexual abuse issues.  Bachelor’s degree in Psychology, Sociology, Social Work or other related degree with minimum of 12 credits in Humanities or Social Services. Fluency in Spanish is essential.

 

 

GROUP LEADERS – ASSURE PROGRAM

Provide PART-TIME, during the Fall, instruction to youth (K-8) in the areas, including but not limited to, recreation, fine arts, camping, and academics.  Maintain individual classroom attendance, liaison with supervisor regarding attendance and discipline.  Enforce program regulations and maintain program boundaries, as well as developing rainy day games for the children to enjoy.  Requirements:  Candidate must present documented evidence of skill-set in a particular area of instruction.  At least a H.S. Diploma or GED, plus 18 months experience with youth in recreation, fine arts, camping or academic experience or  2 years of college experience plus 1 year of work experience with children.

BEHAVIORAL SPECIALIST – Wraparound

Responsible for the development and implementation of behavior plans for clients as assigned. Supervision of the TSS in terms of behavior plan implementation. If identified as team leader, responsible for effective and reciprocal team communication.  Requirements:  Master’s Degree from an accredited university in a clinical mental health discipline, such as Psychology, Counseling, Clinical Social Work, Psychiatric Nursing, or Marriage and Family Therapy.  Degree must include foundations of behavioral health an the provision of mental health services (e.g. individual group and family therapy, cognitive behavioral therapy, psychodynamic therapy, crisis counseling, abnormal psychology, psychopathology, assessment, child development, personality theory, psychotherapy lab, clinical practicum).  For the degree to count as a mental health degree, the degree must contain at least twelve (12) credit hours in core behavioral health coursework as defined above. At least one (1) year verified post-Master’s experience providing mental health treatment including psychotherapy, counseling, and assessment with children and families.

 

MOBILE THERAPIST - BHRS

Provide intensive therapeutic services to children and families in a setting in the form of assessments, individual, group, and family therapy utilizing CASSP principles. Services are provided in an environment other than a provider agency or office; therapy is provided in the home, foster home, and other community settings. Work with a multidisciplinary team to ensure ongoing communication of treatment effectiveness.  Requirements:  Master’s Degree from an accredited university in a clinical mental health discipline, such as Psychology, Counseling, Clinical Social Work, Psychiatric Nursing, or Marriage and Family Therapy.  At least one (1) year verified post-Master’s experience providing psychotherapy/counseling to children.

 

CLINICAL COORDINATOR – BHRS

Responsible for clinical and administrative supervision of the Fee for Service employees (Behavioral Specialist, Mobil Therapist), and admiistrative supervision of the Therapeutic Staff Support. The Clinical Coordinator works in collaboration with the assigned module Case Coordinator and Payroll Processor to ensure the submission and accuracy of all clinical and payroll paperwork is in accordance to CBH guidelines.  The Clinical Coordinator works in collaboration with the Clinical Administrator , Payroll Administrator and Human Resources to ensure all client related data, authorization information, staffing needs and disciplinary procedures are clearly and effectively communicated and expedited. Requirements:  Licensed mental health clinician or Master’s degree from an accredited univeristy in a clinical mental health discipline (Master’s degree must include clinical coursework in the foundations of behavioral health and the provision of mental health services). At least 2 years of verified post-Master’s experience providing mental health treatment including psychotherapy, counseling and assessment. Strong organizational skills, ability to hold staff accountable for paperwork and disciplinary issues excellent communication skills, and proficiency in Microsoft Office software.

 

 

CONSULTATION & EDUCATIONAL SPECIALIST                                                       

Act as a liaison between clients, family members, schools and various agencies to develop and maintain effective linkages to ensure effectiveness and continuity of care.  Requirements:  Bachelor’s degree in an area of human services or social services and a minimum of one year experience in CASSP system.  Experience with children and families with emotional behavior, mental health needs or experience with children and families with abuse and neglect needs.  A valid driver’s license is preferred.  20% travel.

 

THERAPEUTIC SUPPORT STAFF

Daytime Hours - Responsible for directly implementing the treatment plan and interventions suggested by evaluator and tracking frequency, intensity, and duration during each contact with the client.  Weekly supervision and one monthly team supervision required for successful outcomes with the case assigned.  Requirements:  At least one year experience working with children and adolescents in the behavioral health field or education setting.  Bachelor's degree in Social Sciences, Psychology, Sociology, Behavioral Health, or Human Services Degree.  Experience with Behavioral Health Diagnosis a plus.  Reliable transportation required

Conduct individual and group activities to encourage learning and develop social interaction skills while parents attend parenting workshop. Provide a safe, interesting atmosphere for young children.  Requirements:  High School diploma or GED equivalent.  Prior experience caring for children in group settings a plus. Knowledge of child social, and skills development methods and techniques.  Ability to understand and follow specific instructions and procedures.  Knowledge of play, reading, and quiet-time activities for children. Child supervision skills.  Ability to provide a supportive and caring environment for children. Ability to communicate effectively orally.  Travel: Only as needed (up to 20%) to get supplies/food.  Hours/Days Required:  Tuesday’s 5 pm – 8pm and Saturday’s 9:30 am – 12:30 pm.

 

 

 

CASE MANAGER In Home Protective Services – SCOH                                         11/8/07

Provide home-based services to families involved with Department of Human Services.  Main responsibilities include, but are not limited to family assessment, monitoring of children’s safety and well-being, family focused social work, structured interventions to promote life skills development, advocacy, and coordination of services.  Requirements:  Bachelor’s degree in a related field with minimum of two years of experience in social services is required.  This position entails some evening hours and weekends as needed.  Approximately 80% of the work is in the community and requires travel throughout Philadelphia.

Serve as the primary therapist to the children and families participating in the program.  Function as the team leader and provide/coordinate community- based mental health services along with the Mental Health Worker.  Responsible for dDeveloping  and implementing systemically- based interventions utilizing structural family therapy, cognitive-behavioral therapy, behavior modification, psycho-educationpsycho-education, and other researched-based approaches.  Requirements:  Graduate degree in psychology, counseling, family therapy, social work, nursing, theology with an emphasis in pastoral counseling, or rehabilitation counseling with a minimum of 12 graduate credit hours in therapy and/or counseling, and two years experience in child and adolescent mental health services.

Provide clinical and case management services to children in Community Residential Rehabilitative Services.  Acts a liaison between client, foster family, family members, schools, and various agencies to develop and maintain effective linkage to ensure treatment, and/or service effectiveness and continuity of care.  Provide therapy to client and his/her family.  Requirements: Master’s degree with 2 years experience in social work, psychology, counseling or related mental health fieldfield, or Bachelor’s degree with 5 years experience.  Master’s preferred.  Experience in home-based work with families, a plus. .  Must pass criminal, child abuse history clearance and medical clearance.  Up to 80% of work entails home-based and community-based work, which includes travel to each home/site.

FOSTER PARENT EDUCATOR

Foster Parent Trainer (FPT) is responsible for pPresenting pre-service and ongoing trainings to potential and current foster parents to ensure compliance with regulations.  FPT will need to aAlter/update trainings as needed,and create new trainings as needed.  , beBe familiar with foster care and host home regulations regarding trainings and contact foster parents to ensure compliance with training.  FPT will also need to tKeep track of parent trainings.  FPT will need to bBe available to present trainings on weekends and evenings .  Requirements:  and be CPR/First Aid certified or willing to attend classes to obtain certification.  Therapeutic Behavior Management certification is also required
